Change of Designated Premises Supervisor
Welcome to our Vary Designated Premises Supervisor (DPS) instruction form.
If you complete the details required we will then make a DPS variation application on your behalf.
To make your experience quicker and easier have the premises licence and relevant personal licence with you.
Our fee for making the application is £120 increasing to £150 if we have to chase you for information or documents, plus vat and disbursements. The fee to the council is £23 for each application.
Once you have submitted the details to us via applications online we will contact you to confirm your instructions and arrange for payment and then prepare and submit the application.
